Patient Forms to Record Pre- & Post-procedure pain intensity
Below are links to free downloadable documents & tools used by Mark Laslett in clinical practice. These documents are forms for patients to complete before and after interventional diagnostic or therapeutic interventions so that accurate measures of pain intensity are recorded. Patient forms are PDFs to be printed and given to patients to complete. Excel spreadsheets are also downloadable free of charge and may be used to calculate percentage pain intensity changes over time. Pain intensity values (0-100) are recorded in the Excel spreadsheets with all calculations automatically charted in attractive layouts that can be copied and pasted into reports and other documents. Print these PDF forms on a good quality inkjet or laser printer in landscape mode. Each horizontal VAS should print out to be more than 90mm (3.54 inches) long at least, and preferably close to 100mm long (3.94 inches). Measure the line length in mm and use that in the Excel spreadsheets so that the pain intensity scores are more accurately percentalised.
